WHAT YOU NEED TO KNOW
Shoulder pain is one of the most common injuries seen in both active individuals and the general population. Whether it develops in the gym, during sport, at work, or even while sleeping, shoulder pain is rarely caused by just “one injured muscle.” A proper shoulder assessment helps identify why the pain developed, what structures may be overloaded, and which movement patterns are contributing to symptoms.
WHAT MAKES THE SHOULDER SO COMPLEX?
The shoulder is the most mobile joint in the body, which is great for throwing, lifting, swimming, gym training, and overhead movement — but that mobility comes at the cost of stability.
Unlike the hip, the shoulder sits in a very shallow socket (similar to a golf ball on a tee) and relies heavily on:
- Rotator cuff muscles
- Scapular control
- Ligaments
- Posture and movement coordination
If one part is not functioning properly, the rest of the system compensates.
WHY DOES YOUR PHYSIO CHECK YOUR SHOULDER BLADES?
The scapula acts as the foundation for shoulder movement. If it does not move properly, the rotator cuff and surrounding tissues often become overloaded.
Poor scapular control may contribute to:
- Pain lifting the arm
- Weakness
- Clicking
- Impingement symptoms
- Reduced overhead performance
This is why physios often assess posture, shoulder blade movement, and muscle control during an assessment.
WHY DO PHYSIOS TEST THE NECK TOO?
Not all shoulder pain actually comes from the shoulder.
The neck and nervous system can refer symptoms into the:
- Shoulder
- Arm
- Shoulder blade
- Hand
This is why assessments may also include:
- Neck movement testing
- Strength testing
- Reflexes
- Nerve tension testing
If you have tingling, numbness, burning pain, or symptoms travelling into the arm, the neck may be contributing.
WHAT DOES A SHOULDER ASSESSMENT INVOLVE?
A proper shoulder assessment usually includes:
- Movement assessment
- Range of motion testing
- Strength testing
- Rotator cuff assessment
- Scapular assessment
- Special tests
- Functional movement testing
Rather than relying on one test alone, clinicians combine all findings together to understand the bigger picture.
WHY IS EARLY ASSESSMENT IMPORTANT?
Many shoulder problems become worse when people continue training through pain or ignore movement limitations.
Early assessment can help:
- Identify strength deficits
- Improve movement quality
- Reduce re-injury risk
- Restore overhead function
- Guide safe return to training and sport
The goal is not simply to reduce pain, it is to restore confident, strong, pain-free movement.
